Meet the Team
Kiara Lee-- MSW, CCM
Owner and Care Manager
Kiara Lee has over 30 years of experience providing care management services and support to older individuals and their families. Kiara received her master’s degree from the University of California at Berkeley with a specialization in Gerontology.
Kiara has provided services to elders in a variety of settings including retirement housing, adult day health, medical home health, hospice and a crisis hotline before establishing Integrity Care Management in 2012. With experience in case management, counseling,
long-range planning and crisis intervention, Kiara has the skills to help elders and their families to cope with the challenges—and appreciate the blessings—of the aging process.
Gail Zwetsch has had a career serving adults, families, and children. She has her bachelor’s degree in Psychology/Sociology from State University of New York Geneseo, and her master’s degree in Counseling Psychology from Meridian University. She has worked with seniors as a case manager, financial services specialist, and therapeutic recreation specialist.
Gail has a warm and nurturing manner and is skilled at bridging the needs of clients, family members, and service providers to ensure that elders can age with comfort, grace, and dignity. She recognizes the losses and emotionally charged decisions that can come with aging and provides compassion, wisdom, and respect to navigate these challenges. Originally from the East Coast, Gail has called the Bay Area home for 25 years and still enjoys exploring the West.
Gail Zwetsch, BA, MA
Tekla Shelton has a Bachelor of Arts in Social Work from San Jose State and a Master of Public Administration from San Francisco State. Tekla has focused her entire career in geriatrics, including in previous positions as an assisted living administrator, a wellness coordinator, an ombudsman, a private care manager and a case manager for low-income seniors.
Her love of helping seniors started when she cared for her grandma as a college student. She is passionate about senior care and helping people live life to their fullest with dignity. Tekla is a Bay Area native and is married with two sons in who keep her busy.
Tekla Shelton, BSW, MPA
Nancy Patocchi Lund, BSW
A lifelong local of Marin County, Nancy Lund began her work with elders at age 17 at a Novato skilled nursing facility. She graduated with a Bachelor of Arts in Social Welfare from California State University, Chico. She started at Novato Community Hospital and then worked as a Medical Assistant in a busy medical office.
Nancy then worked for 17 years in the Long Term Care Insurance business. She worked supporting claimants and worked her way up to Senior Benefits Analyst and trainer of new employees. She was named "Employee with Heart" out of over 100 employees which matches her personality and temperament to this day. Currently semi-retired, provides Life Enhancement Services to Integrity Care Management clients.
